Procedure for Applying for an LGV Licence

In order to attend practical training and subsequent LGV practical test, certain criteria must be met by the student, these are as follows:

Undertake LGV medical

Obtain LGV provisional licence (cat C)

Sit and pass theory and hazard perception test
E.P. Training Services can arrange (for companies only) all of the above at no extra cost.
Medical

In the first instance, students must undertake a medical to prove they are physically fit to drive an LGV. This can be arranged through the student's own GP but please bear in mind that this can cost anything up to £120. A lot of money for a 20 minute medical!. Alternatively, we have an arrangement with a local practitioner based in Woking. The Maybury Surgery carry out LGV medicals at a cost of £50 per person. Should you wish to book with the Maybury Surgery, they can be contacted on 01483 728 757. Please note. The Maybury Surgery is only open between certain times. Monday to Friday 9am till 11am and 4.30 pm till 6pm (till 7pm on Monday and Friday). (Wednesday 9am till 10 am only)The doctor undertaking the medical is required to complete form D4 of the LGV provisional application form. The medical should last no longer than 20 minutes and once the medical is completed the doctor will hand back the completed D4 form to the student.
Applying for Provisional Licence

Next, students must apply for the correct LGV provisional entitlement to be put on their licence.
A provisional LGV licence application form can be sent to the student requesting him/her to complete certain parts. Form D2 is required to be completed. (Please ensure the application form is read carefully before it is completed, especially the section asking which licence is wanted, the "C" Box should always be ticked. Please call EPTS if in any doubt.)
Identification (where requested) must be sent with the provisional application form. The identification required will depend on whether the student holds the new photo card licence (please read guide carefully). If you have a new photo card licence then section 6 (confirming your identity), section 7 (signing your photograph) and a photograph of you need NOT be completed / sent. Please note. The LGV provisional application is free of charge.
When complete, post the forms and identification to the DVLA in the brown pre-addressed envelope. Alternatively, they can be forwarded to E.P. Training Services first who will check that everything is correct before forwarding to the DVLA. The student should expect the licence back within 7-10 days. If you currently have a photo type licence, then the documents that need to be sent to the DVLA are:

The counterpart (paper) section of your licence.
While waiting for the licence to come through, it would be highly advisable for you to start reading The official LGV Theory Test book and Hazard Perception DVD. The LGV Theory Test book is available from E.P. Training Services or all major newsagents at a cost of £14.99 (plus £5 postage and packing). The Hazard Perception DVD is also £14.99 (plus postage) and available from E.P. Training Services. Please note, you can also visit the DSA (Driving Standards Agency) at www.dsa.gov.uk and purchase the above publications.
Theory Test

As soon as the student receives his/her licence back from DVLA, please either contact E.P. Training Services on 01372 450800 or visit www.dsa.gov.uk to arrange and book the theory and hazard perception test.
As of the 4th August 2008, the LGV theory and hazard perception test is changing. The theory and hazard perception are now two separate examinations and can be sat independently of each other. To pass the theory test you are required to answer 100 multiple choice questions. You must get at least 85 correct to be awarded a pass. The theory test exam lasts 1 hour and 45 minutes. Their are a number of tools available to help you prepare for the test. You can purchase "The official LGV Theory Test Book", you can also purchase "The Official LGV Theory Test" CD ROM. Alternatively visit www.dsa.gov.uk and follow the links for the mock theory test exam.
The hazard perception test consists of 19 hazard clips, you are required to identify as and when hazards appear on the screen. It is highly recommended you acquire The Official Hazard Perception CD ROM. The required passmark is 67 out of 100.
If you fail the hazard perception test but have passed the theory test, then you only resit the hazard perception test, and vice versa.
There are numerous test centres around the country, so you shouldn't far to travel to take the theory and hazard perception test. Once a booking has been made, confirmation of date, time and location will be sent to you. Alternatively you can also book the theory test yourself by calling the DSA on 0300 200 1122, (1,2). You will need the student's driver number (on licence) and a credit/debit card to pay for it. The fee is £35.00 for the theory test and £15.00 for the hazard perception test. The student will receive the results straight after the test and receive his/her certificate of pass/failure.
Once the student has passed his/her theory test it is possible to have an assessment (optional) and then take the practical training.
